Job Description


  Have you walked the walk in accessing HIV care, mental health or substance abuse services.  Does spending your days providing support so that others can walk their walk and know they are supported?  Broward House has a role for you if you answered yes.  We are looking for a Care Navigator to join our team.

In addition to other duties, this position is responsible for

  • Meeting clients with hope and encouragement 
  • Listening to client to understand their goals, strengths and challenges 
  • Participate in creating a plan with the client and other professionals to meet the clients goals
  • Get creative in interventions to help clients follow their plan
  • Advocate to community resources on behalf of the client
  • Facilitate groups to encourage self sufficiency and support building 
  • Provide education on HIV Care and reaching an undetectable viral load 
  • Create an opportunity for clients to define their own lives in a safe accepting environment  

Skills / Requirements

  • Two years’ experience in Social Services, or equivalent experience
  • Lived experience similar to those we serve 
  • Knowledge of HIV/AIDS, preferred 
  • Knowledge of Substance Abuse and Mental Health Symptoms and basic interventions
  • Effective verbal and written communication skills
  • Solution focused approach to conflict and challenges
  • Moderate to Advanced Computer Skills
  • Ability to work independently with minimal direction
  • Valid Florida Drivers License and ongoing proof of insurance required

  • SOAR Training
  • Valid Florida Drivers License and ongoing proof of insurance required
  • Jail Clearance for outreach purposes

Must pass all drug and background screening requirements, including a level 2 federal background and motor vehicle screening
