We are currently looking for Housing Case Managers to join our dynamic client services department who is effective in working with diverse populations.  In addition to other duties, this position is responsible for:

  • Meet directly with clients to assess the strengths and needs to meeting housing and health goals.
  • Work along side client to develop a plan to meet their goals.
  • Link clients with services to maintain an undetectable viral load and secure safe housing.
  • Teach life skills to maintain safe stable housing
  • Reach out to landlords and other housing providers to develop relationship to support Broward House clients
  • Conduct groups with client to build support 
  • Work in a multidisciplinary team to meet the needs of client in the most effective way
  • Provide intervention and support for people impacted by addiction and mental health challenges

 

Skills / Requirements

  • Bachelor’s degree in a social service or health related field Or Licensed Practical Nurse with 2 years case management/social services experience
  • HIV/AIDS knowledge preferred
  • Effective verbal and written communication skills
  • Ability to establish and maintain effective working relationships with employees, collaborating agencies and the public
  • Ability to demonstrate and model culturally competent behavior
  • Moderate to Advanced Computer Skills
  • Ability to work independently with minimal direction
  • Knowledge of Substance Abuse and Mental Health Symptoms and basic interventions, preferred
