Does work as a part of team excite you?   You then may want to be a part of our Transitional Housing Program in supporting those living with HIV impacted by co-occurring mental health and addiction.

In addition to other responsibilities, this position will have the opportunity to:

  • Complete an integrated assessment of individual client’s strengths, needs, abilities and preferences.
  • Develop relationships for transitional housing placements.
  • Develops an integrated and collaborative individualized care plan with a transition component.
  • Provides the appropriate level of individual and group therapeutic intervention to support the stabilization and progression of clients.
  • Provides linkage and navigation for health care, supports services and housing stability based on individualized needs.
  • Develops and prepares groups to meet client needs, including transition and after care.
  • Provides supports to other disciplines in the residential program including but not limited to nursing, assessment, milieu, and Behavioral Health
  • Meets with internal and external interdisciplinary treatment team members to coordinate client services and documents outcomes of meetings, including hospitals, health department and medical providers.
  • Submits all documentation in an accurate and timely manner, including electronic records Achieves and maintains program established productivity levels.
  • Engages clients in a strength-based supportive manner providing re-directions and guidance as necessary.
  • Other duties as assigned.

Skills / Requirements

SKILLS / REQUIREMENTS

  •  HIV 500 (provided upon hire)
  • Valid Florida Driver’s License and ongoing proof of insurance required.
  • HOPWA training modules (provided upon hire)
  • Case Management and Therapy experience preferred.
  • A minimum of two years' experience which may include internships, preferred.
  • Substance abuse treatment or residential setting experience preferred.
  • HIV/AIDS knowledge preferred.
  • Proficient computer skills.
  • Bilingual preferred.
  • Knowledge of Stages of Change, Trauma Informed Care and Harm Reduction
  • Must have a bachelor’s degree in social work, Health or Human Services from an accredited university or commensurate experience.
  • Valid Florida Driver's license and ongoing proof of insurance required and ability to provide own transportation.
  • Ability to work independently with minimal direction.

Must pass all drug and background screening requirements, including a level 2 federal background and motor vehicle screening
